Documents Required for Proposal

  • Completed signed/dated Outfitter & Guide Application
  • Photos of the complex
  • Currently valued insurance companies loss runs for the current policy period plus three prior years for accounts over $10,000
  • Advertsing brochure and website information
  • Waiver of liability form
